Locating Your PSA ID

You recently received a Consent Form containing your PSA ID. Locate the PSA ID in the bottom right-hand corner of your Consent Form, under Question 9 (“What do I have to do in order to participate in the lawsuit?”) in your Notice, or at the bottom of your Consent Form.
